Marie Reidelbach put together a fantastic Copyright and Plagiarism guide for the University of Nebraska Medical Center. The "Recommended Guidelines" she lays out in the fair use area are clear, concise, and obviously guidelines as opposed to hard and fast rules. The section on obtaining permission is clearly laid out, & links to more resources abound.

Kate Borowske created a great Copyright and Fair Use guide for the Bush Memorial Library. She very smartly used a theme of "Tightrope Walk or Tug-of-War", both in the way she presents content and in her images and page layout. She also brings in great videos and links to more resources.
Copyright and Fair Use Guide at NYU Poly
Ingrid Redman created a great Copyright and Fair Use guide for NYU Polytech. Her layout is clear, her images are great (!), and she includes great videos and web links. The page on "Copyright Guidelines for Faculty" is particularly awesome - clear, concise, and well integrated with the rest of the guide.
